Workplace Injuries
What is Workplace Injuries?
Did you have a bad slip, trip, or fall in the workplace? Maybe you were hit by a falling object and were injured? Performing your standard workplace duties and were hurt by muscle strain, or even a repetitive straining injury?
Above are some of the most common injuries that can happen at the workplace. Unfortunately, it is not unheard of for workers to be injured in the workplace. We aim to have our clients back to a healthy condition as efficiently as possible, so it’s vital that we take the proper steps.
If you have been injured at the workplace, your first priority is to get appropriate medical attention. Please notify the workplace supervisor immediately so that they can fill out the mandatory incident report form right away to start a claim for you.
We are fully registered with the Workplace Safety and Insurance Board (WSIB).
Procedure
- Injured in the workplace? Please immediately notify your employer/workplace, so the next steps of them filling out the appropriate paperwork in order to start a claim for you may proceed.
- Your next step, please call and set up an appointment with your doctor, or visit a walk-in clinic.
- If necessary, our trained physiotherapists will be able to assess and examine you promptly after the injury even if you haven’t been able to see your doctor yet.
- We can fill out the necessary paperwork for WSIB at the Alleviate Physiotherapy clinic to promptly begin the medical portion of your claim (Form 8 or a Functional Ability Form (FAF)).
- It is also possible for us to communicate with your employer/workplace to work out a plan for getting you back to work, as safely as possible, and with the assistance you need.
- Additionally, we are able to talk with your doctor as well, about any concerns, if you wish for us to do so.

- WSIB Form 7 is what your employer has to complete and submit within 3 days of learning about your injury
