What is Vestibular Dysfunction?
Vestibular Dysfunction (Vertigo) causes a sensation of spinning or dizziness, which can be caused by a problem in the inner ear, brain, or sensory nerve pathways. Some of the most common causes of vertigo include inner ear infections, Benign Paroxysmal Positional Vertigo (BPPV), and Meniere's disease. Symptoms of vertigo may include dizziness, nausea, vomiting, and loss of balance. Physiotherapists trained in vestibular rehabilitation play a Key role in effective recovery, maintenance and prevention of worsening of this condition. What happens in (Vestibular Dysfunction) Vertigo?
The vestibular system includes the inner ear and parts of the brain that help control our balance and eye movements. Damage to this system can result in the improper processing of signals received by your eyes and inner ear which is known as Vestibular Dysfunction.
Vertigo is the most common Vestibular Dysfunction that causes an illusion of motion and a sense of spinning or dizziness. Dizziness is a collection of symptoms including light-headedness, imbalance, and a sensation of passing out.

- A feeling of spinning or dizziness

- Difficulty walking or standing still
- Nausea or vomiting
- Sweating
- Ringing in the ears (tinnitus)
- Hearing loss
- A sense of fullness or pressure in the ear
- Visual disturbances, such as double vision or light headedness

What diagnostic tests do you conduct for Vestibular Dysfunction?
Our physiotherapists and Vertigo experts use a combination of techniques to diagnose Vestibular Dysfunction. The first step in the process is a thorough client history, in which our physiotherapists will ask detailed questions about the symptoms, such as when they started, how often they occur, and what makes them worse or better. We also ask about any related medical conditions and medications, as well as the client's physical and functional abilities.
After the client history, we perform a physical examination, which typically include the following:
- Assessment of visual and auditory acuity
- Assessing the client's balance and gait
- Assessing the client's range of motion and strength
- Assessing the client’s reflexes
- Assessing the client’s coordination and reaction time
Based on the results of the client history and physical examination, we use specific tests and measures to further evaluate the client's vestibular function.
How can Physiotherapy help in the treatment of Vertigo or Vestibular Dysfunctions?
Physiotherapy can be an effective treatment for vestibular dysfunction. The specific treatment plan will depend on the underlying cause of the Vestibular Dysfunction and the client's individual symptoms. Most common ways vestibular physiotherapists help with Vestibular Dysfunction are:
- Vestibular Rehabilitation Exercises: These are highly specific exercises that aim to retrain the brain to use other sensory information, such as vision, to maintain balance and reduce symptoms of dizziness. These exercises can include head and eye movements(VOR), balance training, and gait training.
- Habituation exercises: These exercises help the brain to "get used to" the abnormal signals coming from the vestibular system and reduce symptoms.
- Canalith repositioning procedures: These are specific maneuvers “Not an exercises” that are done by Physiotherapist with special interest in Vestibular ( certified vestibular therapist) to help move loose particles in the inner ear (canaliths) that are causing symptoms of benign paroxysmal positional vertigo (BPPV).
- Neuromuscular re-education: These exercises help to improve muscle strength and coordination to enhance balance and stability.
- Education and counseling: Physiotherapists expertise can provide education and counseling on how to manage symptoms, such as how to perform activities of daily living safely and how to avoid triggers that can worsen symptoms.

Do you use Infrared Goggles for diagnosis and treatment of Vestibular Dysfunction?
Infrared goggles “considered gold standard” are a type of equipment that can be used in the evaluation and treatment of Vestibular Dysfunction. They are typically used in conjunction with other diagnostic tests and measures, such as video Head Impulse Testing (vHIT), which is a test that uses infrared goggles to track the movement of the eyes in response to specific head movements.
The goggles have infrared cameras that track the movement of the eyes and record the data. This data is then analyzed by our physiotherapists, to determine if there is a problem with the Vestibulo-Ocular Reflex (VOR), which is the reflex that helps to keep the eyes stable during head movement. The VOR is controlled by the vestibular system, which includes the inner ear and the brainstem, and is responsible for maintaining balance and coordinating eye and head movements.
It's important to note that infrared goggles are typically used as part of a comprehensive diagnostic module and treatment plan for Vestibular Dysfunction. They are not used as a standalone treatment. They can provide important information that can be used to develop an individualized treatment plan for all our clients.
